This includes arthritis, diabetes, heart disease, depression, obesity, pain and more. Gold Coast Health Plan (GCHP) would like to invite members to join the free Chronic Disease Self-Management Program (CDSMP). The CDSMP is a self- management education workshop for people with chronic health issues. THE WORKSHOP: ❱  ❱ Is once a week for 2½ hours. ❱  ❱ Lasts six weeks. ❱  ❱ Will help you gain self- confidence in taking charge of your health symptoms. ❱  ❱ Teaches you how to make an action plan and about pain and fatigue management. You will also learn how to build communication skills, work with your doctor on medication usage and more. Committed to your satisfaction Gold Coast Health Plan (GCHP) wants you to be happy with your health care and services. But sometimes you might not be, and GCHP wants to hear about it. If you are not happy with a provider or the provider’s office, it is best to talk to GCHP first. The same goes if you are not happy with your experience in a hospital or other facility. If you have a problem, GCHP is here to help. You can file a grievance or an appeal by calling Member Services at 1-888-301-1228 . If you use a TTY, call 1-888-310-7347 . You may file a grievance with GCHP if you are not happy with: ●  ● The care your doctor is providing or how you were treated in the office. ●  ● The ability to get the care you need. ●  ● The services you get from GCHP. You may file an appeal with GCHP if services have been changed or denied. Once you let GCHP know about your grievance or appeal, someone will look into it. If you are getting a bill for services covered by GCHP, call the billing office at the number on your statement. Let them know what insurance you have and ask them to send a claim to GCHP. Gold Coast Health Plan (GCHP) wants to help you stay healthy! GCHP has partnered with HMS Eliza to provide member outreach. The program will remind you to complete preventive care ser vices and help you make appointments with your primar y care doctor. Preventive care includes services such as getting your flu shot, Pap tests, mammograms and well-child visits. You may get a call from a phone number that is out-of-the-area (267 area code).
