Here is how it works: ●HSA will first try to automatically fill out your renewal using information from electronic data sources. ●If you have Medi-Cal through the Social Security Administration (SSA) because you get Supplemental Security Income (SSI), you have a different process. SSA will let you know if they need any information from you. If HSA or SSA needs information from you to renew your benefits, they will send you a redetermination packet or ask for verification. ●It is important to return it by the due date. If you do not, your Medi-Cal benefits may end. If you have moved since you enrolled, call HSA at 1-888-472-4463, Monday through Friday, from 8 a.m. to 5 p.m., and give them your new address. Well-baby visits for your growing baby What can the doctor review at a well-baby exam? ●Health history. ●Physical exam. ● Height, weight, body mass index (BMI) and head circumference. ● Hearing and vision. ● Screening for lead exposure. ●Physical activity. ● Healthy eating habits. ● Behavior with family and groups. ●Give vaccines. ● Screening for physical and mental development. ● Health education and safety. ● Apply dental fluoride varnish. Vaccines A baby will get vaccines during their well-baby visits. Many will be given together to lower the number of times the child gets a shot. Talk to the doctor about which vaccines are needed. Developmental growth screening A vital part of your child’s routine well-child visits is their developmental growth screening. This involves a written and verbal chat with the doctor about how your child learns, plays, speaks, acts and moves. Your doctor will also see how your child is during the visit and ask the parent and/or guardian questions about how they are doing at home. How often should a baby see their doctor for a well-baby visit? A baby should have at least eight wellbaby visits by 30 months. Visits should take place at the following intervals: ● First three to five days of life. ●One month. ●Two months. ●Four months. ●Six months. ●Nine months. ●12 months. ●15 months. ●18 months. ●24 months. ●30 months. New member incentive: Lead screening in children Exposure to toxic lead can cause lifelong learning and behavioral issues. Gold Coast Health Plan (GCHP) wants all parents and guardians of children 2 years of age and younger to have their child tested for blood lead levels to understand their risk. Screenings are recommended at 12 months and 24 months as part of a routine well-child visit. GCHP offers members a $25 gift card for completing a blood lead test before the child’s second birthday. The lead screening member incentive flyer can be found on goldcoasthealthplan.org under “For Members” and “Member rewards.” It must be completed and mailed or faxed to GCHP after your child completes their blood lead test to receive the gift card. Some examples of when you should pay are: ●If you have Medicare, but they do not pay for your medicine. Or if you have a co-pay, Medi-Cal cannot help you pay. ●If you don’t have Medicare yet but qualify for it, Medi-Cal cannot help you pay. ●If you have a Share of Cost, you must pay that amount each month before Medi-Cal can pay. If you paid for something that is not covered, you can talk to your doctor about changing it or sending a special form called a Prior Authorization (PA) to ask Medi-Cal to cover it. Members who qualify will be able to meet with a registered dietitian, who can provide nutrition consultations and tailor the meals to meet their health needs. Who is eligible? You may be eligible for Medically Supportive Foods if you: ●Have chronic health issues such as diabetes, heart disease, cancer, and kidney failure. ●Are being discharged from the hospital or a skilled nursing facility or are at high risk of a hospital stay or nursing facility placement. ●Have many care coordination needs. Qualifying members must also live within Ventura County. Medically Supportive Foods do not replace or affect your CalFresh benefits. Asthma is a long-term health issue, and it takes ongoing care to keep it under control. A written asthma action plan is one of the first things you and your doctor should work on. This form will help you know what medications you should take daily, how to use them, and how to handle worsening asthma symptoms. Visit the Gold Coast Health Plan website for an asthma action plan to fill out with your doctor. Talk to your doctor if you have any questions. Most people with asthma have two types of medications prescribed: 1. Long-term control medications are taken daily to control your asthma and keep you from having attacks. 2. Quick-relief medications are used when you need fast relief from asthma symptoms and to treat an asthma attack. Call your doctor right away if you are having an asthma attack and if your symptoms do not get better with your quick-relief medications. If you have trouble breathing, walking, and talking, you should call 911. Routine visits If you have asthma, you should meet with your doctor often to review asthma medications and your health in general, especially if your asthma symptoms are worsening, or you are using your quick-relief medications more often.
