Members who qualify will be able to meet with a registered dietitian, who can provide nutrition consultations and tailor the meals to meet their health needs. Who is eligible? You may be eligible for Medically Supportive Foods if you: ●Have chronic health issues such as diabetes, heart disease, cancer, and kidney failure. ●Are being discharged from the hospital or a skilled nursing facility or are at high risk of a hospital stay or nursing facility placement. ●Have many care coordination needs. Qualifying members must also live within Ventura County. Medically Supportive Foods do not replace or affect your CalFresh benefits. Asthma is a long-term health issue, and it takes ongoing care to keep it under control. A written asthma action plan is one of the first things you and your doctor should work on. This form will help you know what medications you should take daily, how to use them, and how to handle worsening asthma symptoms. Visit the Gold Coast Health Plan website for an asthma action plan to fill out with your doctor. Talk to your doctor if you have any questions. Most people with asthma have two types of medications prescribed: 1. Long-term control medications are taken daily to control your asthma and keep you from having attacks. 2. Quick-relief medications are used when you need fast relief from asthma symptoms and to treat an asthma attack. Call your doctor right away if you are having an asthma attack and if your symptoms do not get better with your quick-relief medications. If you have trouble breathing, walking, and talking, you should call 911. Routine visits If you have asthma, you should meet with your doctor often to review asthma medications and your health in general, especially if your asthma symptoms are worsening, or you are using your quick-relief medications more often.
