Ask for and receive care coordination. 17. Make an advance directive. 18. Have an adult represent you with GCHP, once the Plan receives and validates the appropriate permissions from you. 19. Be free from any form of control or limitation used as a means of pressure, punishment, convenience or revenge against you. 20. Make suggestions to GCHP about your member rights and responsibilities. GCHP wants to hear from you. 21. Freely use these rights without changing the way you are treated by GCHP, providers or by the state health care department.  Member Responsibilities As a GCHP member, you have a responsibility to: 1. Carefully read the GCHP Member Handbook and other materials you may receive to understand how to use your benefits and what steps to follow when you need care. 2. Always show your GCHP member identification card and any other active health insurance cards you may have when getting services. 3. Promptly let the Medi-Cal eligibility office and GCHP know of any changes to your name, address, phone number and other health care coverage. If you get Supplemental Security Income (SSI), call the Social Security Administration (SSA) office to make changes. All agencies need to have your correct information. 4. Select a PCP within the first 30 days of being a GCHP member. 5. Make an appointment with your PCP within the first 120 days of being a GCHP member for a health evaluation. 6. Treat GCHP staff, health care provider(s) and their staff in a respectful and courteous way. 7. Be on time for your appointments and inform your doctor’s office if you must cancel or reschedule, and do so at least 24 business hours in advance. 8. Tell your medical provider about all of your medical condition(s), health care needs and any medications you are taking to get the best care plan for you. 9. Follow the care plan and orders for care that you have agreed upon with your doctor. 10. Ask your provider questions if you do not understand something. 11. Contact your doctor if you have problems with the care plan. 12. Call your doctor first when you need health care. 13. Use the emergency room only in cases of an emergency or as directed by your doctor. 14. Follow up with your PCP after getting care at an emergency facility or urgent care center. 15. Talk to your doctor about things you can do to improve your health. Take part in health care programs that keep you healthy. 16. Request interpreter services at least five working days before a scheduled appointment. 17. Call your doctor or pharmacy at least three days before you run out of medicine. 18. Pay for your monthly Share of Cost (if you have one) and for Medi- Cal non-covered services. 19. Report fraud, waste and abuse to GCHP. You can do this without giving your name by calling GCHP’s hotline at 1-866-672-2615 24 hours a day, seven days a week. 20. Call GCHP Member Services at 1-888-301-1228 if you do not know how to use your benefits or if you have any problems with services needed or received. If you use a TTY, call 1-888-310-7347 .
