WINNING HEALTH 4 Care Management: We’re here for you The Gold Coast Health Plan (GCHP) Care Management team is here for you. The team will work with you, your family and your health care providers to get you the care and services you need. You can call with concerns about your treatment and problems with your medical equipment or supplies. You can also call for help getting appointments. The Care Management team can also help you: ❱  ❱ Understand your condition and your doctor’s plan for you. ❱  ❱ Learn about your medications. ❱  ❱ Get connected with other agencies. ❱  ❱ Coordinate your care between your doctors. ❱  ❱ Schedule appointments. If you use a TTY, call 1-888-310-7347 . Do you have questions about your Medi-Cal benefits? Gold Coast Health Plan (GCHP) offers three benefit information meetings each month. The Plan can answer any questions you have. At the meeting, you will learn about: ❱  ❱ Medi-Cal benefits and how to use them. ❱  ❱ Your rights and responsibilities as a GCHP member. ❱  ❱ Choosing and changing your Primary Care Provider (PCP). ❱  ❱ Getting dental, vision and behavioral health ser vices. ❱  ❱ Transportation to your appointments. ❱  ❱ Resources to improve your health. Gold Coast Health Plan (GCHP) values your health! To reward your healthy behavior, GCHP has created two new programs. Members who qualify can get a gift card for getting these preventive care exams: ●  ● Annual Well-Care Exam ■  ■ Members 3 to 21 years of age who have a well- care visit in 2020 can get a $15 gift card to Target, Walmart or Amazon. ●  ● Cervical Cancer Screening ■  ■ Members 21 to 64 years of age who have a cervical cancer screening (Pap test) in 2020 can get a $25 gift card to Target, Walmart or Amazon. How can you participate? 1. Schedule your qualifying preventive screening exam. 2. If you did not get a form in the mail, download the Member Rewards form from the website, GoldCoastHealth Plan.org . Click on Member Resources, then Member Rewards. 3. Take the form with you to your doctor visit. Fill out the top half of the form and have your doctor fill out and sign or stamp the bottom part. 4.
