WINNING HEALTH 4 Have you received a bill from a provider? Medi-Cal members should not be billed for covered ser vices. If you receive a bill from a Medi-Cal provider for a covered ser vice, call Gold Coast Health Plan (GCHP) as soon as possible at 1-888-301-1228 . If you use a TTY, call 1-888-310-3660 . When you call, have a copy of the bill with the following information: ❱ Your GCHP identification number. ❱ The date of the medical ser vice. ❱ The name of the doctor or hospital. ❱ The amount of the bill. Always carry your GCHP member ID card with you. Show your card at every doctor visit to make sure services are covered and to avoid getting billed by mistake. Vision services benefit Vision care is provided through Vision Ser vice Plan (VSP) providers. All members can have eye exams every two years. Members who have been diagnosed with diabetes should get an eye exam every year. When you call for your appointment, tell your provider you have diabetes. Gold Coast Health Plan (GCHP) covers: ❱ A routine eye exam once every two years. GCHP may approve fur ther ser vices as medically necessary. ❱ Eyeglasses (frames and lens) once every two years. ❱ Contact lenses when required for medical conditions such as aphakia, aniridia and keratoconus. Don’t let your child miss these vital preventive health visits You may have received a reminder in the mail from Gold Coast Health Plan (GCHP) to make an appointment for your child’s preventive care services. Make an appointment today if your child needs any of these free health care services: ● Well-check visit: A checkup on your child’s health and development. ● Immunizations: All needed shots to prevent diseases and keep your child healthy. ● Blood lead testing: Routine screenings at 12 and 24 months of age, and up to 6 years of age if not done before.
