This packet will include a Health Information Form Member Evaluation Tool (HIFMET). This form will let GCHP know your current health care needs so we can help you get the care you need. Medi-Cal renews once a year unless you have a change that affects your eligibility during the year. Renewals are done by the Ventura County Human Services Agency (HSA), not by Gold Coast Health Plan (GCHP). Here is how it works: ●HSA will first try to automatically fill out your renewal using information from electronic data sources. ●If you have Medi-Cal through the Social Security Administration (SSA), you have a different process because you get Supplemental Security Income (SSI). SSA will let you know if they need any information from you. If HSA or SSA needs information from you to renew your benefits, they will send you a redetermination packet or ask for verification. ●It is important to return it by the due date. If you do not, your Medi‑Cal benefits may end. If you have moved since you enrolled, call HSA at 1-888-472-4463, Monday through Friday, 8 a.m. to 5 p.m., and give them your new address. When you sign up, you will get help to make healthier food choices, move more, and lower your risk of type 2 diabetes. Members who enroll can get an activity tracker, a wireless scale (with online programs), and help from a health coach. The program is available in English and Spanish at no cost to you. INCENTIVES OFFERED Child and adolescent well-care visits: Members 3 to 21 years of age should complete a well-child visit once a year with their doctor. This visit is often called a physical, well-child, or checkup. For completing this exam, members will get a $25 gift card. Cervical cancer screening (Pap test or well-woman exam): Female members 21 to 64 years of age should get a Pap test with their Primary Care Provider or OB-GYN every three to five years, based on the type of screening done. For completing this exam, members will get a $50 gift card. Breast cancer (mammogram or x-ray of the breast): Members ages 40 to 74 should get a mammogram every two years. Early detection can save lives. For completing this exam, members will get a $50 gift card. HOW TO APPLY FOR YOUR INCENTIVE Members can earn one gift card per year for each qualifying program. 5 Member Services: 1-888-301-1228/TTY 711 Screening for colon health It is vital to get screened for colon cancer. Symptoms often do not show up until the cancer is advanced. Regular screenings make it more likely to find cancer early when cancer may be easier to treat. Your risk for colorectal cancer gets higher as you get older. It is recommended to start screening at age 45 for people at average risk. It should continue through age 75. Several types of tests can be used. Some tests can be completed at home or the doctor’s office. Talk to your doctor about which option is best for you. No matter which test you choose, the most important thing is to get screened regularly. Call your doctor today to schedule your colon cancer screening. Enhanced Care Management and Community Supports: What you need to know Enhanced Care Management (ECM) and Community Supports (CS) are a part of the Medi-Cal transformation that aims to break down barriers to health. They offer benefits and services that help meet the needs of Medi-Cal beneficiaries. ECM is an in-person benefit for people with many health care needs. A care coordinator can help get people extra services to stay healthy. They also make sure all health providers work together on each person’s health goals. Care coordinators can also find and connect members to other health services in the community. CS services are offered by Gold Coast Health Plan (GCHP) to help with problems like getting housing and food. GCHP has these services available at no cost for members who need them most. These services include: ●Community transition services: Helps members live in their residence after leaving a nursing facility. ●Home modifications: Changes are made to homes to make them accessible. ●Housing deposits: Funding to help set up housing. ●Housing tenancy: Help to keep safe and stable housing. ●Housing transition navigation: Help to find and get housing. ●Medically supportive food: Healthy meals that are delivered to homes. ●Personal care and homemaker services: In-home services to help with activities like bathing, feeding, and going to medical appointments. ●Recuperative care: Short-term housing and care after leaving the hospital. ●Respite services: Temporary relief for those who care for members. ●Short-term post-hospitalization housing: One-time temporary housing for members who do not have housing and are leaving the hospital or a treatment facility. Children 18 years and younger need to get the vaccines from their doctor. RSV vaccines are not for everyone. Please check with your doctor or pharmacist if you need one or have any questions.
