Chronic Disease Self-Management Program Do you have a chronic health issue, such as arthritis, diabetes, heart disease, depression, obesity, or chronic pain? GCHP invites you to join the free Chronic Disease Self- Management Program (CDSMP). You will learn how to make an action plan, about pain and fatigue management, how to work with your doctor, about medication usage, and more. Diabetes Prevention Program The Diabetes Prevention Program (DPP) is a benefit that was created by the Centers for Disease Control and Prevention. This program works with those with prediabetes to help prevent type 2 diabetes. Having prediabetes means your blood sugar is higher than normal, but you do not have diabetes yet.
